Calculadora online

Bom dia! Criei uma calculadora online seguindo este vídeo tutorial.

Na administração do site, ao trabalhar com o módulo webforms tudo correu bem. Os problemas surgiram ao trabalhar com js. 

Por favor, me digam o que e onde preciso registrar para vincular o arquivo js à página onde ficará a calculadora e como nomear esse arquivo, e onde ele deve ficar.
