Theming fields in webform

Hello) Could you please tell me how to implement a field in webform that would be used for entering a phone number in a predefined format? I saw an implementation like this on the website http://www.valize.com.ua/product/89531-sumka-osen-zima-2013-2014-deeporill, where when you click on the phone input field, a form appears for filling out. However, I couldn't figure out how it was done. There are additional CCK modules, but they would create a feedback form, which is not suitable for this case. Thank you in advance)
