External form - embedding

Hello. Could you please tell me how to correctly insert an external form into a specific block?

I have created a form (it is located separately in a folder, along with all the js and css files).

I want to place it on the main page in a specific block. How can I implement this correctly so that its functionality (defined in the js files) is preserved?
